Professional statement of need 

Once you have filled out the professional statement of need please fax it along with a CAF form to the county of financial responsibility. We also ask the you fax is a copy of the professional statement of need as well.

Combined application form (CAF) 

This form is necessary to determine eligibility for county services and funding.

DHS release of information 

Please complete a release of information and fax a copy to Hotel Boarding so that we may communicate with your county about the services and funding you receive. Include a copy to the county when you submit your professional statement of need
